Supplier Company Contacts Form
Telecommunication suppliers give contact information to the PUC on who to contact regarding such items as financial data, tariffs, customer service and annual reports. The form includes, name, title, address, phone numbers and email address for each contact.
Every jurisdictional telecommunications utility must complete and submit a Report of Outage Form to the Commission when they experience an unscheduled service interruption event that affects 2,500 or 5%, whichever is less, of their total customers for 6 or more consecutive hours. The Form is due within 10 working days after the last customer affected is restored. 52 Pa. Code Chapter 67.1.
Public Utility Security Planning & Readiness Self Certification
All utilities under the reporting requirements of 52 Pa. Code §§ 57.47, 59.48, 61.28, 63.36, 65.19, 29.43, 31.1 or 33.103 shall annually file the form by February 28th. The PDF form may be filled out electronically, printed out (or printed out and filled in by hand), and must be mailed to Secretary of the Commission, Pennsylvania Public Utility Commission, Attention Doreen Trout, P.O. Box 3265, Harrisburg, Pa. 17105-3265. The forms may no longer be emailed to the Commission due to security concerns.
Utilities that have been directed to file an adoption supplement must file a separate form for each tariff adopted. The supplement may be filed on one days notice.
Public Utility Security Planning & Readiness Self Certification-Frequently Asked Questions
The PUC adopted a Public Utility Planning and Readiness Self Certification Rulemaking order at Docket L-00040166. The rulemaking resulted in the creation of a new regulation at 52 PA Code Chapter 101. The attached file contains answers to frequently asked questions concerning the form that is to be filed in compliance with 52 PA Code Chapter 101.4.
Regulations for Abandonment of Local Telephone Service
All local service providers intending to abandon local telephone service must fulfill the requirements at 52 Pa. Code, Chapter 63.306-308.
Application for Approval of Abandonment or Discontinuance of Service

2018 TRS Annual Access Line Report
An annual reporting that all local exchange carriers must file that summarizes the amount of access lines the company had in service each year. Attachment A is included in this Excel Form. This is required by 52 Pa. Code § 63.37.
52 Pa. Code § 64.201 – Annual LEC Report (ILEC & CLEC)
An annual report by all ILECs and CLECs that contains residential account information for the previous year as listed in 52 Pa. Code § 64.201. The report is due within 90 days after the end of each calendar year.
